EARNED PRIZE FORM AND INFORMATION

Once you have completed your travels please fill out the following form.  You will need to email photos of pages 4-7 in the Shop Guide to quiltingacrossthedakotas@gmail.com.  

You can also return to any participating shop for assistance.  

*If you received your 10 Shop Earned Prize and did not get to 20, you don't need to fill this out.  

**All prizes will be mailed AFTER the event  (by the end of the Sept.)
