EARNED PRIZE FORM AND INFORMATION

Once you have completed your travels please fill out the following form.  You will need to email photos of pages 4-7 in the Shop Guide to quiltingacrossthedakotas@gmail.com.  

You can also return to any participating shop for assistance.  

*If you recieved your 10 Shop Earned Prize BUT did not get to 20, you do not need to send photos.

*All prizes will be mailed AFTER the event  (by the end of the Sept.)

Form will be Available 7/23
